Since she was young, my patient Sophia (not her real name) always had a BMI that placed her in the “overweight” category. Her mother put her on a strict diet when she was 12 years old. Doctors had counseled her to lose weight since she was in high school. Unsurprisingly, the constant focus on the number on the scale had taken a toll on her sense of self-worth. When she came to see me as a new patient in her 60s, she expected to hear the same refrain.
But when I looked at her complete health picture, I saw something different. Her blood pressure was a normal 118/76. Her cholesterol profile was excellent—optimal LDL, high HDL, and normal triglycerides. Her A1C and fasting glucose were in perfect ranges. She exercised four days a week, ate a primarily plant-based diet rich in whole foods, and slept well. By every measure except BMI, Sophia was metabolically healthy. The number on the scale told me almost nothing about the actual state of her metabolic health.
Sophia’s story illustrates a fundamental problem with how we think about health in the US. We’ve become obsessed with weight as a proxy for wellness, missing the much more important question: How well is your body actually functioning?
Recent data suggests that over 88% of American adults have some degree of metabolic dysfunction. Yet paradoxically, we continue to focus almost exclusively on weight and BMI—metrics that can be profoundly misleading. Someone can have a “normal” BMI and terrible metabolic health if they’re sedentary, eat poorly, or carry most of their fat around their organs. Conversely, someone like Sophia can have a higher BMI and be remarkably healthy if they’re muscular, active, and carry their weight in metabolically safer locations.
The real story of metabolic health happens beneath the surface, in how your body processes energy, responds to food, manages stress, and regulates inflammation. These processes determine whether you’ll develop diabetes, heart disease, fatty liver disease, and countless other conditions—regardless of what you weigh.
The Warning Signs We’re Missing
In my practice, I see the earliest signs of metabolic dysfunction long before lab values show prediabetes or diabetes. Patients describe energy crashes in the afternoon, feeling shaky if they skip meals, difficulty concentrating after eating, or that peculiar “wired but tired” sensation. Sleep disruption is enormous. Many people wake between 2-4 AM as their body releases stress hormones to compensate for blood sugar dips during the night. Others have lost touch with normal hunger and fullness cues entirely, eating either nothing until they’re ravenous or continuing until they’re uncomfortably stuffed.
These subjective experiences matter as much as any lab value because they reflect what’s happening at the cellular level. When your body struggles to regulate blood sugar effectively, you feel it in your energy, your mood, your sleep, and your relationship with food—often years before a doctor diagnoses you with anything.
The Stress-Metabolism Connection
What makes metabolic dysfunction particularly insidious is how it feeds on itself (no pun intended!) through multiple systems. Chronic stress, for instance, creates a perfect storm for metabolic problems that has nothing to do with what you’re eating. When we’re stressed, cortisol rises, which directly interferes with insulin’s ability to move glucose into cells. This creates insulin resistance even in people eating perfectly healthy diets.
I see this constantly in my practice—patients whose metabolic markers improve dramatically not when they change their diet, but when they are able to address chronic stressors like caregiving responsibilities, work pressures, or financial anxiety. The body interprets chronic stress as a threat requiring immediate energy availability, so it keeps glucose elevated and promotes fat storage around the midsection. Sometimes the most important “prescription” for metabolic health isn’t a medication or a diet; it’s helping someone set boundaries at work or find better ways to cope.
The sleep connection compounds this problem. Poor sleep is essentially metabolic poison, disrupting every hormone involved in glucose regulation. When you’re not sleeping well, your cortisol levels rise, your insulin sensitivity plummets, and your body’s ability to process carbohydrates deteriorates. Many of my patients experience what looks like insulin resistance that’s actually sleep deprivation in disguise. Address the sleep apnea or insomnia, and suddenly their glucose regulation normalizes without any dietary changes.
Beyond the Scale
Metabolic health requires looking beyond weight to a constellation of factors: blood pressure, lipid profiles (not just total cholesterol, but the ratio of HDL to triglycerides), Lp(a) and ApoB biomarkers, inflammatory markers like C-reactive protein, fasting glucose and A1C, and perhaps most importantly, body composition—how much muscle versus fat you carry, and where that fat is stored.
Visceral fat, the kind that accumulates around your organs, is metabolically dangerous even in people who look thin. Subcutaneous fat, the kind you can pinch, is relatively benign. Muscle tissue, meanwhile, is metabolically active—it’s like building a glucose disposal system. This is why strength training emerges as one of the most powerful interventions for metabolic health, often more impactful than losing weight.
When patients ask me what actually improves metabolic health, I focus on five key areas:
Strength training two to three times weekly to build metabolically active muscle tissue. The benefits appear remarkably quickly—within weeks, patients see improvements in glucose regulation and insulin sensitivity.
Adequate protein intake at every meal—aiming for 20-30 grams—which helps stabilize blood sugar and promotes satiety.
Movement after meals. Even a 10-minute walk can significantly blunt the glucose spike that follows eating.
Sleep quality. During deep sleep, your brain clears metabolic waste products and your body restores hormonal balance. Chronic sleep deprivation impairs virtually every aspect of metabolic function. If you snore, feel tired despite adequate sleep time, or wake frequently during the night, addressing these issues can have profound metabolic benefits.
Stress management—not as a luxury but as a metabolic intervention. Whether through therapy, meditation, setting boundaries, or simply learning to say no, finding ways to reduce chronic stress activation directly improves insulin sensitivity and glucose regulation.
As I discussed in my recent newsletter about GLP-1 medications like Ozempic, pharmaceutical tools can help restore normal satiety signals and address underlying metabolic dysfunction for some people. But these medications work best as part of a comprehensive approach that includes the interventions above. They create a window of opportunity for healthier behaviors, but the behaviors themselves—strength training, adequate protein, stress management, quality sleep—are what maintain metabolic health over the long term.
From Shame to Science
Another one of my patients, a middle aged woman who had struggled with her weight since pregnancy, told me recently that when she stopped trying to be thin and started trying to be strong, everything changed. She slept better, her blood sugar normalized, and she had energy to play with her kids. She admitted to me that her body changed more than it ever did when she was obsessing about the scale.
This is the paradox of metabolic health: when you focus on the markers that actually matter—glucose regulation, insulin sensitivity, inflammation, stress hormones, sleep quality—the outcomes you want often follow naturally. But when you focus solely on weight, you can miss profound metabolic dysfunction or, worse, damage your metabolic health through extreme restriction, chronic stress about food, and the shame that comes from failing to achieve an arbitrary number.
The goal isn’t to ignore weight entirely. Significant changes in weight can indicate important shifts in metabolic health, and excess adipose tissue does create inflammation and hormonal disruption. But weight is one data point among many, and often not the most important one.
If you want to assess your metabolic health, start by paying attention to those subjective markers I mentioned: your energy patterns throughout the day, whether you sleep through the night, how you feel after eating, whether you can recognize true hunger and comfortable fullness. Then work with your doctor to get the right lab tests—not just a lipid panel, but A1C, inflammatory markers, and a comprehensive metabolic panel.
Focus on behaviors that improve metabolic function regardless of whether they change your weight: building strength, eating adequate protein, managing stress, prioritizing sleep, moving after meals. These interventions work by addressing the underlying cellular dysfunction that defines poor metabolic health, not by forcing your body into an arbitrary size.
Perhaps most importantly, work on removing the moral judgments about food and bodies that create shame and stress. Shame itself activates stress responses that directly worsen metabolic health. Sometimes healing your relationship with food and your body is as important as anything you put on your plate.
To understand how metabolically healthy you are, the question isn’t “What do you weigh?” but rather “How effectively does your body regulate blood sugar, respond to insulin, manage inflammation, and maintain energy throughout the day?”
For Sophia, understanding this distinction was transformative. She stopped trying to shrink her body and started focusing on keeping it strong, well-rested, and metabolically flexible. Her labs remained excellent. Her energy stayed consistent. And the mental space she’d spent worrying about her weight became available for everything else in her life that actually mattered.

All I can say is BRILLIANT! THIS IS THE MESSAGE DOCTORS NEED TO HEAR! Also, yoga is my go to for years. I have been a teacher and am a regular practitioner… easy to adapt all yoga with age. I have been “technically “ overweight since my 40’s after being a small size up until then. Now as I approach 70 all my markers are good. The most insidious issue is the cultural poison that infects with ideas of “appropriate“ and often unattainable weight numbers. Long overdue, it’s time to get off that merry go round. We have much higher purposes.
